The first medication, mifepristone, works by blocking progesterone, a hormone that plays a significant role in supporting pregnancy. By blocking progesterone, the uterine environment changes in a way that interrupts the progression of the pregnancy.

The second medication, misoprostol, is generally taken 24–48 hours later. Misoprostol causes uterine contractions that expel the pregnancy tissue and uterine lining from the body.

These medications affect the body in significant ways and involve more than a single pill or isolated event. Understanding the process, potential risks, and recommended medical evaluations beforehand can help women make informed decisions regarding their health.

The Importance of Pre-Abortion Care

A medical decision involving pregnancy also involves understanding the overall health and wellbeing of the woman experiencing it. Pre-abortion medical care serves an important purpose: it helps identify conditions that may affect treatment, safety, and future health.

Confirming Pregnancy Location

One of the most important questions to answer is where the pregnancy is developing. An ultrasound can identify whether the pregnancy is located inside the uterus. In some cases, a pregnancy develops outside the uterus—most commonly in a fallopian tube. This is called an ectopic pregnancy. An ectopic pregnancy cannot continue normally and requires medical treatment. Because abortion medication does not resolve an ectopic pregnancy, identifying its presence is an important part of protecting a woman’s health.

Determining Gestational Age

Medical abortion is intended to be used within a specific pregnancy timeframe. Although estimated due dates can be calculated based on a woman’s last menstrual cycle, ultrasound provides a more accurate understanding of gestational age and can help determine whether medication use aligns with medical guidelines.

Evaluating Pregnancy Viability

Ultrasound also provides information regarding pregnancy development. Some pregnancies end naturally through miscarriage, sometimes before a woman realizes it has occurred. Understanding what is happening medically can help avoid unnecessary procedures and ensure women receive appropriate care.

Reviewing Other Health Considerations

Medical history, medications, and STI screening can provide important information regarding overall health and safety. Many infections can occur without obvious symptoms, and identifying these factors beforehand helps women receive more complete care and treatment if needed. These evaluations are not intended to create barriers. Rather, they help ensure women have the information needed to make healthcare decisions with greater understanding and confidence.

Understanding Possible Complications and When Medical Care Is Needed

Like many medical interventions, abortion medications can involve side effects and potential complications. Women may experience bleeding, cramping, nausea, vomiting, fatigue, and other physical symptoms as their bodies respond to the medications. In some situations, additional medical care may become necessary.

Women should seek immediate medical attention if they experience symptoms such as:

  • Symptoms that feel significantly outside expected recovery patterns

When a Woman Has Second Thoughts

Pregnancy decisions can involve intense emotions, and emotions do not always follow a predictable timeline. Some women feel confident in their decisions. Others feel uncertain. Some begin the process and later realize they want to continue the pregnancy. For women who have taken the first medication, mifepristone, but have not yet taken the second medication, there may be a time-sensitive intervention available known as Abortion Pill Reversal (APR).

Understanding Abortion Pill Reversal

Abortion Pill Reversal is an intervention that attempts to counteract the effects of mifepristone through the use of supplemental progesterone. Because mifepristone works by blocking progesterone, APR seeks to increase progesterone levels in an effort to support continuation of the pregnancy before the second medication has been taken.

Timing plays an important role. The process is generally considered most effective when initiated as quickly as possible after taking mifepristone, although there have been documented cases in which treatment began later.

Progesterone itself is not new to medicine. It has been used for many years in pregnancy care, including support for pregnancies considered at risk for miscarriage. For women who experience regret or uncertainty after taking the first medication, immediate medical guidance can be important because the available window for intervention is limited.
